Who is eligible for home health care?

Is Your Child Eligible for Care Under GAPP?
GAPP is a Medicaid program for medically-qualified children under the age of 21. Children of families eligible for Medicaid can apply for GAPP at any time and must meet the same level of care necessary for admission to a hospital or skilled nursing facility. GAPP is managed by the Georgia Department of Community Health (DCH).
